What is your go-to approach for depression counseling?

I use an eclectic approach to treat depression, pulling from cognitive behavioral therapy, dialectical behavior therapy, somatic therapy, and a systems perspective. I find this approach works well, as it allows me to tailor therapy to each client’s individual needs. For clients who have a history of trauma, it often helps to clear the trauma first and then develop skills to help mitigate the depression. Not all people experiencing clinical depression are amenable to antidepressant medication. In such cases, therapies that address thoughts, feelings, and behaviors may be their preference.

What tools do you teach in depression counseling?

I teach the following tools in depression counseling:

  • Education about depression and its treatment
  • Coping/soothing skills to help manage stress and emotional flooding
  • Thought management skills to help identify negative self-talk and unhelpful patterns of thinking
  • Emotional regulation skills to help reduce the emotional rollercoaster
  • Somatic skills that help the body shift mood
  • Interpersonal skills to help with getting needs met
  • Self-care skills to help with depression recovery and prevention of future relapses

I know a client is making meaningful progress in depression counseling when they are finding the energy to participate in the activities they once enjoyed. I will hear a shift in how clients talk and what they say. It is often subtle and may be overlooked by the client initially.

What can clients do in their personal time to supplement depression counseling?

Clients can supplement their time in depression counseling with consistent self-care. Providing for the body’s needs helps it support the brain’s mental processes. A nutritious diet provides the nutrients the body needs to produce neurotransmitters and cellular energy. Restorative sleep is essential for good mental functioning. Engaging in a physical activity that elevates your heart rate is a natural means of improving mood and reducing stress. If you have not had a medical checkup with your primary doctor, schedule one, as poor sleep and depression are sometimes the symptom of a medical problem. If you are isolating inside your house, get social! There is a high chance you will not feel like doing any of these things, but do them anyway. That is one way to fight depression naturally between therapy appointments. Cultivate a mindset of respect for yourself and your body.

What should someone do to prepare for starting depression counseling?

In preparation for your first counseling session, reflect on the history of your depression. Try to identify whether you have experienced depression in the past, or when the first time you recall feeling depressed for a week or more was. Have you experienced recurring depression? How old were you when you first noticed you felt depressed? How does depression affect your life? Does it keep you from doing things? Does it make you do things you typically would not do? What makes it feel better/less intense? What makes it feel worse? What have you already tried? Is it better or worse at certain times of the day? Just be curious about your depression and approach it from an objective stance. As a therapist, the more information a client can provide on their symptoms and functioning, the better I am able to figure out what type of depression we are working with and how to treat it.
