The information you need to provide to obtain medical records from Thriveworks depends on what you are looking for. This could include:

  • Client’s first and last name
  • Client’s date of birth
  • Client’s city and state
  • Name of the client’s clinician
  • Any written documentation that goes along with the request (e.g., custody agreement, ROI, and PHI)

Types of medical records requests we fulfill

Individual Client Medical Records

This type of request is for a client to receive their own records directly. Prior to initiating a ticket, you’ll need to have completed an ROI. If you need to fill one out, please contact your clinician or support@thriveworks.com, and they will send you one through the Client Portal.

Co-Counseling Client Medical Records

This type of request is for a client that was part of couple or family counseling to get a copy of their records. Prior to initiating a ticket, you’ll need to have each member of your co-counseling group (Client Unit) sign an ROI. If you or someone else in your Client Unit needs to fill one out, please contact your clinician or support@thriveworks.com, and they will send you one through the Client Portal.

Child Medical Records

This type of request is for a client under the age of 18. Prior to initiating a ticket, because the child is under 18, all parents and/or guardians with the legal right will need to complete an ROI or provide us with a custody agreement if one is in place in order for us to release the documents. If you need to fill one out, please contact your clinician or support@thriveworks.com, and they will send you one through the Client Portal.

Insurance Medical Records Requests​

This type of request is for records or an audit of records from an insurance company. Please include a copy of the request and any other information pertaining to the request. Please include the Thriveworks location, clinician name, and client’s full name and date of birth for proper verification.

Medical Records Request From a Third Party

This type of request is for someone requesting records of a Thriveworks client, apart from an insurance company or subpoena, that is not the client themselves. This may be an attorney, employer, etc. Prior to initiating a ticket, you’ll need to have the client complete an ROI.

If you need the client to fill one out, please contact the client to reach out to their clinician or support@thriveworks.com, and they will send the client one through the Client Portal. If the client attended co-counseling sessions, we need an ROI from every member of the Client Unit. If the client is a child under the age of 18, we would need all required parents (s)/guardian(s) to sign an ROI. Supply the clinician with the custody agreement if applicable.

Subpoena Duces Tecum

This type of request is for a subpoena for medical records. Prior to initiating a ticket, you’ll need to have the client complete an ROI. If you need the client to fill one out, please contact the client to reach out to their clinician or support@thriveworks.com, and they will send the client one through the Client Portal. Please include a copy of the subpoena.

FMLA/Short-Term Disability and Similar Medical Records Request

This type of request provides your medical record to an application for FMLA or short-term disability. Prior to initiating a ticket, you’ll need to have completed an ROI. If you need to fill one out, please contact your clinician or support@thriveworks.com, and they will send you one through the Client Portal. If a letter needs to be written or a form filled out, please reach out to your clinician for these first. Please note sending forms or letter requests only to the Medical Records may delay your request being completed. Please make sure to provide a claim number/case number in your request.

When you submit a ticket, please include if you need an ROI or PHI as well as a secure email to reach you at so we can best serve you. Also, please note that all documentation must be uploaded to your Client Portal. Your ticket will be assigned to one of our Medical Records team members — you may provide information or contact them directly through the ticket.

If you need to fax any requests or information, you may send it to 617-431-5497, Attn: Medical Records. A request only needs to be submitted once.

We put our clients' privacy first

We protect your personal health information (PHI) in accordance with the Health Insurance and Portability Accountability Act (HIPAA). We can ONLY release PHI if it falls under one of the HIPAA Privacy Rule exceptions, which includes:

  • The client’s (or clients’) written Release of Information form (ROI)
  • For billing purposes, such as insurance company requests
  • Court-ordered, or in some cases, subpoenas
